Accident vasculaire cérébral survivor: 66 years old

Patient Description:

A 66-year-old female presented with a sudden onset de left-sided weakness, slurred speech, and mouth deviation to le left. She also experienced difficulty in bed mobilité and daily living activities.

 

Case History:

Le patient has had a history de hypertension for le past three years along with dilated cardiomyopathy. She was admitted to le emergency room due to le sudden onset de symptoms and was intubated because de decreased oxygen saturation. Subsequently, she was transferred to le intensive care unit and treated with a variety de medications.

 

Physical Examination Results:

Facial asymmetry was observed with mouth corners deviated to le left. Le patient was unconscious and not oriented. Left-sided muscle tone was decreased. Superficial, deep, and integrated cortical reflexes were decreased, and plantar reflexes were flexed. Cranial nerve examination showed involvement de le facial nerve (sensory and motor parts), balance part de le vestibulocochlear nerve, Broca's aphasia, and dysphagia.

 

Results de pathological tests and other investigations:

Magnetic resonance imaging (MRI) revealed a distinct wedge-shaped intraaxial hypodensity involving le grey and white matter de le right frontal, parietal, and temporal lobes, suggesting acute infarction corresponding to le bilateral MCA territory, accompanied by an 11-mm midline shift.

Treatment plan:

Inpatient rééducation (1 to 2 weeks): Various treatment measures were implemented, such as multimodal auditory stimulation, efficace positioning de le hémiplégique posture, thoracic PNF techniques and bed activity training.

Outpatient rééducation (3 to 6 weeks): Oropharyngeal retraining, facial exercises, sit-to-stand training, scapular PNF pattern, etc. were carried out utilisant Syrebo rééducation gants.
